CLIENT PORTAL:

TO CONFIRM YOUR FIRST RENTAL AND ESTABLISH YOUR ACCOUNT, PLEASE SUBMIT THE FOLLOWING:

1.       MASTER RENTAL AGREEMENT:  The attached Rental Agreement must be filled out and signed by the person responsible for paying for the rental. 
2. CREDIT CARD AUTHORIZATION FORM: All new customers are required to fill out a credit card authorization form for their first rental.  Nashville Lighting & Grip, Inc. will retain the card on file for future rentals unless revoked in writing.  Payment in full is expected at the beginning of the rental period.  New Customers may pay by cash, check, wire transfer or credit card, but a credit card is required to be kept on file and payment will be due prior to pickup.  Please communicate your final payment method to your rental agent. 
3. CERTIFICATE OF INSURANCE (COI):  Please see our Rental Agreement for our insurance requirements.  Nashville Lighting & Grip, Inc should be listed on your insurance certificate as “additionally insured” and “loss payee.”  A valid COI must remain on file at all times during the rental period.

Equipment quantities and availability can only be confirmed after these three requirements have been met.  These documents remain on file for future rentals unless updated or cancelled in writing.  Please let us know if you have any questions.​
