CLIENT FORMS:

WE REQUIRE THE FOLLOWING 3 ITEMS IN ORDER TO CONFIRM YOUR QUOTE
​AND PROCEED WITH YOUR ORDER:

  1.       RENTAL AGREEMENT:  The attached Rental Agreement must be filled out and signed by the person responsible for paying for the rental. 
  2. CREDIT CARD AUTHORIZATION FORM: All new customers are required to fill out a credit card authorization form for their first rental.  The card will be pre-authorized for the full amount of the rental.  When a vehicle is being rented, we authorize an additional $500 for incidentals.  New Customers may also pay by cash, check or wire transfer, but a credit card will be kept on file and payment will be due prior to pickup.  Please communicate your final payment method to your rental agent. 
  3. CERTIFICATE OF INSURANCE (COI):  Please see our Rental Agreement for our insurance requirements.  Nashville Lighting & Grip, Inc should be listed on your insurance certificate as “additionally insured” and “loss payee.”  A valid COI must remain on file at all times during the rental period.
Equipment quantities and availability can only be confirmed after these three requirements have been met.  Documents will remain on file for future rentals.  Please let us know if you have any questions.​
